J Appl Physiol 94: 301–313, 2003. First published September 27, 2002; 10.1152/japplphysiol.00660. 2002.—Little is known of the excitatory mechanisms that contribute to the tonic contraction of the corpus cavernosum smooth muscle in the flaccid state. We used patch-clamp electrophysiology to investigate a previously unidentified inward current in freshly isolated rat and human corporal myocytes. Phenylephrine (PE) contracted cells and activated whole cell currents. Outward current was identified as largeconductance Ca2 -activated K current. The inward current elicited by PE was dependent on the Cl gradient and was inhibited by niflumic acid, indicative of a Ca2 -activated Cl (ClCa) current. Furthermore, spontaneous transient outward and inward currents (STOCs and STICs, respectively) were identified in both rat and human corporal myocytes and derived from large-conductance Ca2 -activated K and ClCa channel activity. STICs and STOCs were inhibited by PE and A-23187, and combined 8-bromoadenosine cAMP and 8-bromoadenosine cGMP decreased their frequency. When studied in vivo, chloride channel blockers transiently increased intracavernosal pressure and prolonged nerve-evoked erections. This report reveals for the first time ClCa current in rat and human corpus cavernosum smooth muscle cells and demonstrates its key functional role in the regulation of penile erection.

The whole-cell perforated patch clamp technique was used to study membrane currents in isolated rabbit corpus cavernosum smooth muscle cells. Depolarization from -80 mV to the range -40 to -10 mV evoked a nifedipine-sensitive Ca(2+) current that was followed by a slower inward current that activated over several hundred milliseconds. The slow current reversed near the Cl(-) equilibrium potentia...

Erectile dysfunction is caused by a variety of pathogenic factors, particularly impaired formation and action of nitric oxide (NO). NO released from nerve endings and corpus cavernosum endothelial cells plays a crucial role in initiating and maintaining increased intracavernous pressure, penile vasodilatation, and penile erection. Classically, these effects are dependent on cGMP synthesized dur...

Penile erection is the ultimate vasculo-tissular response to an integrated and complex series of physiological processes elicited by psychological, neural, and hemodynamic events (Giuliano and Rampin, 2000; Saenz de Tejada, 2002). Upon sexual stimulation, penile erection, occurring in response to the activation of proerectile autonomic pathways, is greatly dependent on adequate inflow of blood ...
